A Study of Positive Rate anti-HCV in Health Screening by a Third-Generation Enzyme-Linked Immunosorbent Assay

Abstract
The detection of an antibody to the hepatitis C virus (anti-HCV) by a third-generation enzyme-linked immunosorbent assay (ELISA) posed a problem of false positive results since the inception of the assay. As the generation of the assay proceeded, the false positive rate decreased. We investigated the positive rate by third generation ELISA tests among visitors for health screening and the concordance rate with the western blot method. Positive rates for anti-HCV were investigated with subjects of health screening tests in Kangbuk Samsung Hospital from November, 2000 to October, 2001. The study included 10,191 subjects (male; 7,039, female; 3,152). A Serum specimen from the suºñects was analyzed using a DONG-A HCV 3.0 kit (Dong-a Pharmaceutical Co., Ltd., Korea). For a confmnatory test, HCV BLOT 3.0 (Genelabs Diagnostics Pte Ltd., Singapore) was used for Western blot in 22 anti-HCV positive samples. The positive rate for anti-HCV was 0.22% (22/10,191). There were no significant sex-differences in the positive rate for anti-HCV; the male population showed a lower positive rate, 0.18% (13/7,039), for anti-HCV than females, 0.29% (9/3,152) (p=0.311). In the 22 anti-HCV positive samples. 9 showed positive, 7 indeterminate, and 6 negative results by the HCV BLOT 3.0 kit. The concordance rate between DONG-A HCV 3.0 and HCV BLOT 3.0 was 72.7%. The third-generation ELISA test by DONG-A HCV 3.0 showed relatively good results in concordance rate with a confmnatory test by HCV BLOT 3.0. As the discordance still exists between methods, we recommends confmnatory tests for samples with positive results on third generation ELISA tests.
